As you begin your treatment with us, we would like to provide the following insurance and financial information to explain your health insurance coverage for your initial consultation and the subsequent diagnostic procedures to determine the underlying cause of your infertility.
The State of Illinois requires the diagnosis and treatment of infertility to be covered at the same benefits as all other conditions. The law does not apply to self insured employers or trusts, contracts underwritten outside of Illinois, employer groups with less than 25 full time employees, or couples that have had a voluntary sterilization. Some of the non-mandated insurance carriers provide only major medical benefits to diagnose the underlying cause of infertility, in which case the following diagnostic processes are typically covered:
- Initial consultation
- Diagnostic evaluation to determine cause of infertility
- Surgery (diagnostic) as needed
A select few insurance carriers will not provide coverage for testing to diagnose the underlying cause of infertility. In these cases, a discounted cash package or financing options are available.
All covered charges will be submitted to your insurance carrier for payment. You will be responsible for all applicable co-pays or deductibles. Thus, we ask you to read through all of our financial information carefully and do not hesitate to contact us with any questions.
At the completion of your diagnostic evaluation you will meet in consultation with your physician to review the results and discuss treatment options. A patient advocate will be available to meet with you after your consultation to answer questions, discuss costs and financial information, and offer financing options associated with your treatment.

Our In Vitro Fertilization (IVF) Fee Reimbursement Program, also referred to as “Slide to Success,” is a way for us to enhance Midwest Fertility Center’s services to its patients. This program enables us to share our success with our patients and by reimbursing certain IVF fees, permitting them to continue and pursue further treatment. Pre-paid Packages: Midwest Fertility Center offers pre-paid treatment packages for Clomid®, IVF, injectable cycles, and various procedures that are not covered by insurance.
